Losartan, metformin, and ranitidine are the three drug products that were recalled due to the detection of NDMA in finished products. The amount of DMA and nitrite in products can provide some information to assess the likelihood of nitrosamine formation. Method development for determining DMA and nitrite in pharmaceuticals was initially based on these three drug products.

Smoking or food intake can also contaminate the oral fluid, causing an altered response 82. Rinsing the mouth before sample collection can help to reduce any residual contamination 40. For most substances, the concentration in this matrix can be estimated on the basis of oral fluid pH in respect to plasma pH, and the pKa of the drug 71, 72. Generally, basic drugs reach higher concentrations in oral fluid than in plasma 73, 74.

The limits of quantification (LOQs) by CE–DAD were sufficient to detect the presence of these analytes in blood after acute exposure. Several of the investigated amphetamines are scheduled in the US and in European Union (EU) 17, 18, 114–116. A GC–MS method, based on derivatization with 2,2,2-trichloroethyl chloroformate, was investigated by Frison et al. 109 for determination of some 2,5-dimethoxyamphetamines (2,5-DMA, DOM and DOB) in plasma, urine and hair samples.

High affinity micro-molar range bromodomain inhibitors I-BET and JQ1 were developed and presented efficiency in vivo against LPS-induced endotoxic shock, bacteria-induced sepsis3, decrease tumor size, and enhanced survival in testis midline carcinoma and myelomas4. DOC is a substituted alpha-methylated phenethylamine, a class of compounds commonly known as amphetamines. Dimethylamphetamine (Metrotonin), also known as dimetamfetamine (INN), dimephenopan and N,N-dimethylamphetamine, is a stimulant drug of the phenethylamine and amphetamine chemical classes. DMA, or dimethoxyamphetamine, is a series of lesser-known psychedelic drugs similar in structure to Amphetamine and to TMA (Trimethoxyamphetamine). They were first synthesized by Alexander Shulgin and written up in his book PIHKAL (Phenethylamines I Have Known and Loved).1 Very little data is known about their dangers or toxicity. It is used in treatment of malarial,173 methemoglobinemia174 and potassium cyanide poisoning.175 The chemical name of methylene blue is 3,7-bis(dimethylamino)phenothiazin-5-ium.

Brompheniramine reacts with maleic acid to produce brompheniramine maleate 124 (ref. 75) (Fig. 30). Triflupromazine comes under the category of phenothiazine derivatives, which are antagonists of the dopamine D1/D2 receptor. It is especially utilized to manage aggressive conduct when psychotic illnesses are experiencing acute episodes. It can also be used to manage moderate to severe pain in some hospitalized patients, severe hiccups, and extreme nausea and vomiting. The chemical name for triflupromazine is N,N-dimethyl-3-2-(trifluoromethyl) phenothiazin-10-yl propan-1-amine. On 26th March 2001, the US FDA approved triflupromazine for the management of schizophrenia.

In LPS-treated cells NF-κB/p65 translocated into the nucleus and pre-treatment with DMA partially suppressed the LPS-induced p65 translocation, suggesting that DMA inhibits the LPS-induced NF-κB activation. Triflupromazine was synthesized by reacting 2-bromobenzenethiol 78 with compound 79 to give compound 80a, which on catalytic reduction nickel give compound 80b. Cyclization of compound 80b yield compound 80c, which on reaction with 3-chloro-N,N-dimethylpropan-1-amine yield 81 triflupromazine56 (Fig. 22). In a study of Cheng et al. of 2020, the prevalence of drugs of abuse detected in Hong Kong from 2016 to 2018 has been investigated on seizures and urine samples. One of the limitations of this study is that analysis of NPS was not included in routine urine testing. Between NPS identified in seizures, there were also 2C-B, 25I-NBOMe, 25I-NBOH and 25C-NBOH 147.

The oral fluid analysis allows to identify the recent intake of a substance, even after few hours from ingestion, depending on its bioavailability 63, 64. According to the European drug report 2021, amphetamines are the second most consumed stimulant drugs in Europe after cocaine 13. Since the early 1980s the term “amphetamine designer drugs” has been introduced to indicate new substances, structurally similar to amphetamine, but with enhanced psychoactive effects.

The chemical name of tetracaine is 2-(dimethylamino)ethyl 4-(butylamino)benzoate. It is synthesized by reacting 4-(butylamino)benzoic acid 218 with ethanol in the presence of HCl to give compound 219, which reacts with 2-(dimethylamino)ethan-1-ol in the presence of NaOEt to form tetracaine 220 (ref. 121) (Fig. 50). Citalopram is a SSRIs antidepressant that is commonly used to cure the symptoms of depression.
Urine sample integrity testing procedure has been published in the US Federal Registry and approved by the US Substance and Mental Health Services Administration (SAMSHA) 52, 53. Another disadvantage of urine analysis is that the presence of drug in urine does not necessarily indicate an use immediately preceding, or even at few hours before using. The presence of a methyl group in alpha position to the amino terminal moiety is typical of the amphetamines’ class, and differentiates them from the corresponding phenylethylamines. The EMA released its guidelines in September 2019 and gave companies 3 years to complete all the mandated steps. The first step is an initial assessment of high-risk products, originally due on March 26.
The Eschweller–Clarke reaction is the conventional synthetic method of preparing DMA in industry, which is accomplished by methylation of alkyl amine by HCOOH/HCHO (ref. 14) (Fig. 2). Both MDA and MDMA are controlled substances in many countries due to their potential for abuse and adverse health effects. MDMA also consistently leads to adrenergic effects, such as increased heart rate and blood pressure, primarily attributed to the release of norepinephrine. 2,5-DMA is the alpha-methyl homologue of 2C-H and could be called “DOH” under the DO naming scheme. As a result of this hassle and confusion, patients are losing faith in the health-care system, she says. “With this loss of trust between patients and their doctors and trust in their drug supply, it’s even harder to convince patients to take medications they need,” she adds.

Bromodomain inhibitors decrease the expression of oncogenes like myc and Runx221,24,25,26,27 and are presently being tested in clinical trials. The activities of JQ1 in suppression of inflammation and the inhibition of osteoclast maturation were sufficient for a short term treatment of periimplantitis22. For long-term treatment of osteoporosis, however, the use of JQ1 could compromise the treatment due to its inhibiting effect on osteoblast maturation21. Tamoxifen is a non-steroidal anti-estrogen that is used to treat breast cancers and to lower the likelihood of developing the disease in those who are at high risk. In these therapies, tamoxifen is administered either as a stand-alone medication or as an adjuvant.
